Submit a claim.
Two ways to submit. Both routes adjudicate to the same plan administrator.
Electronic (EDI)
Submit via your clearinghouse. EDI Payor ID: 52682. Use Payor ID 52682 with your existing EDI clearinghouse (Availity, Change Healthcare, etc.).
By mail
Paper claims and documentation. Claims mailing: PO Box 3012, Milwaukee, WI 53201. Include the patient ID and date of service on every page.
